Overview

A Florida billing partner that works the way Florida payers do.

LRx Healthcare Inc. is a Florida corporation with its office at 10191 West Sample Road, Suite 105, in Coral Springs (Broward County). The office is staffed in person, and billing work is delivered remotely inside your existing practice management and EHR systems — so a practice in Miami, Tampa, Orlando, or Jacksonville is supported the same way as one in Broward or Palm Beach County.

Florida billing has its own rules on top of the national ones. Most Florida Medicaid enrollees are in Statewide Medicaid Managed Care (SMMC) plans, so eligibility checks have to find the assigned plan, not just confirm Medicaid. Florida’s prompt-pay laws set firm deadlines for insurers and HMOs to pay, deny, or contest claims — and for providers to submit them. Our eligibility verification, denial management, and A/R follow-up workflows are built around those rules.

To see where your revenue stands before committing to anything, start with a free billing audit — a diagnostic review of denials, A/R aging, and payer mix that ends with a prioritized fix list. If you are still deciding whether to outsource at all, read in-house vs. outsourced medical billing.

Florida rules

Florida billing rules we work with every day.

These are the state-specific rules that most often decide whether a Florida claim is paid on time. They apply to fully insured, state-regulated plans; self-funded employer plans governed by ERISA, Medicare, and Medicaid follow their own rules.

Prompt-pay deadlines for insurers and HMOs

Under sections 627.6131 (insurers) and 641.3155 (HMOs), Florida Statutes, a payer must pay, deny, or contest a claim within 20 days of receipt when it is submitted electronically, or 40 days on paper. Electronic claims must be paid or denied within 90 days (120 days for paper), and an overdue payment carries simple interest of 12% per year. We track these dates so slow claims are followed up and escalated while they still count.

Six-month filing window for providers

The same statutes require providers to submit a claim to a Florida insurer or HMO within 6 months of the date of service (or discharge, for inpatient care) once the patient’s correct insurer is known, and to a secondary payer within 90 days of the primary’s final determination. Medicare’s limit is 12 months from the date of service. Claims are submitted and rejections worked promptly so none age past these windows.

Florida Medicaid managed care (SMMC)

Florida’s Agency for Health Care Administration (AHCA) delivers most Medicaid coverage through Statewide Medicaid Managed Care plans. Each plan sets its own authorization, timely filing, and claim rules, and network providers generally must also be enrolled with Florida Medicaid. We verify the assigned plan before the visit and handle Medicaid enrollment and revalidation through credentialing.

Limits on overpayment recoupments

Absent fraud, a Florida insurer or HMO generally has 30 months after paying a claim to request an overpayment refund — and 12 months for providers licensed under chapters 458, 459, 460, 461, 466, or 490 (physicians, osteopathic physicians, chiropractors, podiatrists, dentists, and psychologists). Check a recoupment request against these limits before returning money.

Statute summaries are general information, current as of 2026-11-06, not legal advice. Your payer contracts and the current text of the Florida Statutes control.
