Medical Accounts Receivable (A/R) Follow-Up Services

Accounts receivable follow-up services targeting aging buckets, payer escalations, underpayments, and unresolved balances — with clear documentation and recovery trends.

Overview

What A/R follow-up covers.

A/R follow-up is the work of getting every unpaid claim to a final answer: paid, corrected and resubmitted, appealed, or written off for a documented reason. LRx Healthcare works open insurance A/R for medical practices, clinics, labs, behavioral health providers, and pharmacies — prioritizing by age, dollar value, and payer, and contacting payers until each claim is resolved.

Aged A/R is rarely one problem. It is usually many small ones: claims the payer never received, claims pending records, underpayments nobody compared to the contract, and denials nobody appealed. We sort the open balance into those categories first, so effort goes where money is still recoverable — starting with claims that are close to a timely filing or appeal deadline.

Denials found during follow-up move into denial management, posting errors into payment posting, and patient balances into patient calling. To see how your A/R is aging before you commit, start with the free billing audit or read our guide on how to read an A/R aging report.

How it works

How we work an aging A/R.

  1. 1

    Aging bands

Open A/R is split into 0–30, 31–60, 61–90, 91–120, and 120+ day bands by payer, so you can see exactly where money is stalling.

  2. 2

    Prioritization

Claims are ranked by dollar value, remaining timely filing or appeal window, and payer behavior, so the most recoverable money is worked first.

  3. 3

    Claim status check

Status is checked through payer portals, electronic claim status responses (276/277), or a call to the payer — received, pending, denied, or paid.

  4. 4

    Resolution

Each claim gets the action it needs: resubmission, corrected claim, records sent, coordination-of-benefits update, or escalation.

  5. 5

    Appeals coordination

Denied claims are routed into the denial workflow with supporting documentation and tracked to a decision.

  6. 6

    Underpayments

Where your fee schedule or contract is available, payments are compared to the expected allowed amount, and underpaid claims are disputed with the payer.

  7. 7

    Documented contact

Every payer contact is logged with the date, call reference number, and next action, so follow-up never restarts from zero.

  8. 8

    Recovery reporting

Weekly recovery and trend reporting shows what was collected, what is still open, and why.

What we need to get started

  • A current A/R aging report by payer
  • Access to your practice management system and payer portals
  • Fee schedules or contracted rates, for underpayment review
  • A contact for medical records requests
  • A signed Business Associate Agreement before any PHI is exchanged

What your weekly report shows

  • Amount collected and still open, by aging band and payer
  • Claims resolved this period and how each was resolved
  • Underpayments identified and disputed
  • Claims at risk of missing a filing or appeal deadline
  • Write-offs, each with a documented reason

AR Follow-Up FAQ

Common questions about this service.

Quick, direct answers to the questions providers ask most about LRx Healthcare.

We prioritize aging buckets by recovery probability, payer behavior, and dollar value — then escalate stalled claims with documented call notes.
Yes, where requested. Patient balance review and statements can be included alongside payer-side AR follow-up.
Results vary by payer mix and starting AR profile, but most clients see measurable improvement in AR days within the first quarter of cleanup.
A commonly cited target is keeping A/R older than 90 days below roughly 15–20% of total A/R, alongside a net collection rate of 95% or higher. These are widely used industry benchmarks, not guarantees — specialty and payer mix move them.
Yes. Where your fee schedule or payer contract is available, payments are compared to the expected allowed amount and underpayments are disputed with the payer.
Yes. Old A/R is reviewed for what is still within timely filing and appeal limits and worked by priority. Anything unrecoverable is documented so it can be written off with a clear reason.
