Insurance Eligibility & Benefits Verification Services
Eligibility verification services that confirm coverage, benefits, copays, deductibles, and authorization requirements before services are delivered — stopping denials before they start.
Overview
What eligibility verification covers.
Eligibility verification confirms, before the visit, that a patient’s coverage is active, the service is a covered benefit, and you know what the patient owes and what the payer requires. LRx Healthcare runs real-time eligibility and benefits checks for medical practices, clinics, and pharmacies: active status, plan and network, copay, deductible, coinsurance, authorization and referral requirements, and coordination of benefits — documented on the account so your front desk can act on it.
It is the cheapest denial prevention step in the revenue cycle. Coverage errors — terminated coverage (CO-27), the wrong primary payer (CO-22), the wrong payer altogether (CO-109) — are among the most common and most avoidable denials, and a surprise bill weeks after the visit is harder to collect than a known amount collected at check-in. Services that need approval are handed straight to prior authorization, and eligibility denials that still get through are traced back in denial management.
Verification is not one-size-fits-all. For Medicare patients we confirm Original Medicare versus Medicare Advantage; for Medicaid, both state eligibility and the assigned managed care plan; and for behavioral health, whether a carve-out organization manages the benefit instead of the medical plan. Our eligibility verification checklist lists every item front-desk teams should confirm.
Scope: Verification confirms coverage and benefits at a point in time; it does not guarantee payment. The payer still adjudicates the claim against medical necessity, authorization, coding, and timely filing rules — which is why eligibility runs alongside authorization, coding review, and denial management.

How it works
How each patient’s coverage is verified.
- 1
Schedule pull
Upcoming appointments and new patients are taken from your schedule, so verification starts before the patient arrives — not at the front desk.
- 2
Electronic check
Coverage is checked through the HIPAA 270/271 eligibility transaction or the payer portal, with a call to the payer when the electronic response is incomplete.
- 3
Plan & network
Active status, effective and termination dates, plan type, and in-network status for the rendering provider and location are confirmed — the plan, not just the payer.
- 4
Benefits & cost share
Coverage for the scheduled service, visit or frequency limits, copay, remaining deductible, coinsurance, and out-of-pocket maximum are captured so the front desk knows what to collect at check-in.
- 5
Authorization & referral
Prior authorization and referral requirements are checked by payer and CPT or HCPCS code, and anything required is handed to the authorization workflow before the visit.
- 6
Coordination of benefits
Other coverage, payer order, Medicare Secondary Payer status, and behavioral health or pharmacy carve-outs are identified so the claim goes to the right payer first.
- 7
Documentation
Each check is recorded on the account with the date, method, payer reference number, and any issue the front desk needs to resolve with the patient.
- 8
Re-verification
Coverage is re-checked shortly before the visit, and for recurring services such as therapy or ABA at the start of each month and each new plan year.
What we need to get started
- Access to your appointment schedule and practice management system
- Clearinghouse eligibility access and payer portal logins, or the ability to request them
- Insurance card images and intake details collected at registration
- The services you schedule most often, so authorization and referral rules can be checked by code
- A front-desk contact for patients whose coverage needs follow-up
- A signed Business Associate Agreement before any PHI is exchanged
What you can track
- Appointments verified before the visit, by payer and location
- Coverage problems caught before the visit — inactive coverage, plan changes, missing secondary coverage
- Patient cost share captured for collection at check-in
- Authorization and referral requirements flagged and handed off
- Eligibility-related denials, such as CO-27, CO-22, and CO-109, and their trend
- Items waiting on the front desk or the patient
